A Medication Authorization Form is required for any medication administered at school. Click here to download form.

VOLUNTEERS:

All Volunteers at Travis Elementary must complete the Volunteer Registration Form prior to volunteering at our school. Forms MUST be completed every year! Download the Registration form here and return it to your child's teacher or the school office.
