Registration Documentation Needed to Register in Travis Unified

During the school year, registration generally takes place at the school sites.  During the summer months, registration takers place at the District Office.  

This Please bring the following documentation when you register your children:

  • student’s birth certificate
  • up-to-date immunization record
  • proof of residency (current copy of escrow document, copy of lease or rent receipt; PG&E, water, or cable bill.  Families living on Travis Air Force Base may also use a phone bill or a housing letter.)

 

Immunization Checklist

The following immunizations are needed for students entering Travis Unified School District:

Vaccine Required Dose
Polio

4 doses at any age, 3 doses meet requirements for ages 4-6 years if at least one was given on or after the 4th birthday; 

3 doses meet requirement for ages 7-17 years if at least one was given on or after the 2nd birthday

Diphtheria, Tetanus, and Pertussis 5 doses at any age, but4 doses meet requirements for ages 4-6 years if at least one was given on or after the 4th birthday.
Measles, Mumps, and Rubella (MMR) 2 doses* both on or after 1st birthday
Hepatitis B 3 doses

Varicella (Chicken Pox)

1 dose**
* Two does of measles-containing vaccine required.  One dose of mumps and rubella-containing vaccine required: mumps vaccine is not required for children 7 years of age or older
**Effective date 7/1/01.  Physician-documented Varicalla  (chicken pox) disease history or immunity will also meet the Varicella requirement.

  • Any special education Individual Education Plans - Special Day Class, Speech/ Language, Resource
  • 504 Accommodation Plans if applicable
  • GATE Documentation
  • English Learner Documentation

 

Locations Available for Registration

Active Duty Military Families may register at the Airman and Family Readiness Center on the following days:

  • 2 August, 2011 from 10:00 -2:00 pm
  • Families may also register at the District Office over the summer months 2751 DeRonde Dr. Fairfield
  • During the school year, families may register at their home school.
