Pacific Educationrs Insurance Services has low cost-student accident insurance. You may either apply online with a credit card at http://www.peinsurance.com/signup or apply with a paper application.

Athletic Physicals

Those students that still need a sports physical can contact:

Students interested in playing on an extra-curricular athletic team must have proof of a sports physical exam prior to the first practice. Fall try-outs begin the first week of September. Forms are on our website, under “medical forms.” In addition, a Concussion Form must be completed and signed by a parent or guardian.
